Phase 1  Planning (What, When, Where, Who and Who and Why)

Fill in the all of the following and then click on the submit button. You need to be prepared to submit all of this, Phase 1,  at once. You are allowed to change your mind and resubmit another proposal.

5) Who will be your mentor- supervisor? (it is OK to have a parent be your mentor-supervisor) Parents must approve of the supervisor. Your contact person can also be your supervisor. You must include a phone number!
